Mississauga Halton CCAC Issues 2015/16 Annual Report to the Community

 Delivering Constant Care to Mississauga Halton Residents in a Year of Change

ETOBICOKE, ON, June 24, 2016 /CNW/ - The Mississauga Halton Community Care Access Centre (CCAC) is pleased to issue its 2015/16 Annual Report to the Community: Constant Care in a Year of Change for its fiscal year, which ended on March 31, 2016. This report includes highlights on how the Mississauga Halton CCAC's teams cared for 48,000 patients in this year of change, including 1,369 more people than its previous year.

"We look on 2015/16 as a year that we enhanced our ability to 'make people the point of care,' with an unwavering focus," says Caroline Brereton, CEO, Mississauga Halton CCAC.  "We also saw our CCAC take care coordination and effective collaboration to new levels through various initiatives tailored to our community's needs."

The report features four of the Mississauga Halton CCAC's accomplishments from 2015/16:

  • Introduced a rigorous, enhanced Care Coordination Program of Work that ensures patients receive consistent care, which integrates their services where they live and in their own neighbourhoods. (YouTube video shows how this program helps patients and their families.)
  • Developed Ontario's first-of-its-kind Primary Care Provider Database to provide comprehensive, up-to-date physician profiles and enable real time communications with them. Using this tool, Mississauga Halton CCAC engages 76 per cent of all primary care providers across the region.
  • Launched a medical specialist electronic compendium called 'DocSearch' to help primary care providers locate relevant specialists, who are accepting patient referrals.
  • Produced 'Insights' tool, which is an evidence-based, customized dashboard to help care coordinators more effectively manage multiple aspects of each patient's care.

And during 2015/16:

  • 96% of all patients received nursing care through home visits or clinics within five days after their assessment.1
  • 92% of patients with higher, more complex health needs, received their first personal support worker (PSW) visit within five days after their assessment.2
  • 95% of patients surveyed said they would recommend the Mississauga Halton CCAC to family and friends.3

The Mississauga Halton Community Care Access Centre (CCAC) helps adults and children leave hospital earlier to recover at home, remain at home, enter long-term care or die at home, as they wish.  Its experienced care coordinators assess people's needs and collaborate with health care partners to organize in-home and community-based services for residents living in South Etobicoke, Mississauga, Oakville, Milton, Georgetown and other places across Halton Hills.

The Ontario Ministry of Health and Long-Term Care funds the Mississauga Halton CCAC, through the Mississauga Halton Local Health Integration Network (LHIN).
