Integration and collaboration key to better, responsive care
TORONTO, Feb. 26, 2019 /CNW/ - The Alzheimer Society of Ontario applauds Health Minister Christine Elliott on the health-care transformation announced today. As an organization deeply committed to providing local community-based care, the Alzheimer Society looks forward to strengthening our relationships with community partners to ensure the health-care system is coordinated, collaborative and relentlessly focused on seamless care centred around individuals and families living with Alzheimer's disease and other forms of dementia.
The Alzheimer Society provides community-based dementia services across the province through education, counselling and support programs. As partners in the community with other service providers, the Alzheimer Society is confident that further collaboration and coordination will enhance best practices that support Ontarians living with dementia and their care partners.
