New Heart Health Strategy a major step forward in improving cardiovascular care

    Cardiac Care Network dedicated to collaborating to share knowledge and

    TORONTO, Feb. 26 /CNW/ - The Cardiac Care Network (CCN) applauds the
recently-announced Canadian Heart Health Strategy and Action Plan (CHHS-AP)
and looks forward to working with the CHHS-AP Steering Committee as a partner
to advise and serve as a model for programs that improve heart health in
Canada by focusing on enhanced patient-centred services, surveillance and
knowledge infrastructure.
    CCN's ongoing mandate is to advise on the provision of high quality adult
cardiac care services. Through our long-standing programs, Ontario has
implemented a sustainable system to monitor, manage and report on wait times
and quality, and ensure timely and equitable access to advanced cardiac care.
To date, CCN has contributed to advancing cardiac care in Ontario by:

    -   implementing a system of Active Cardiac Access Management for
        advanced cardiac procedures

    -   reducing the total number of wait days for patients awaiting
        cardiac diagnostic catheterization, Coronary Artery Bypass Graft
        Surgery (CABG) and scheduled Percutaneous Coronary Intervention (PCI)

    -   significantly improving regional wait time variation for elective
        cardiac diagnostic catheterization, PCI and CABG

    -   maintaining recommended maximum wait times for diagnostic
        catheterization, PCI and isolated CABG for all centres and all
        urgency categories

    CCN is recognized across Canada as a "gold standard" for a provincial
cardiac system and is well positioned to provide leadership and support the
CHHS-AP Steering Committee by:

    -   leveraging CCN's existing system and infrastructure to monitor
        quality, efficiency, and access and promote best practices and
        standards for cardiovascular care

    -   providing expertise in establishing a system to link higher intensity
        acute care with community and chronic care - ensuring a seamless
        transition for patients between care settings, and facilitating the
        transfer of knowledge between health care providers

    -   sharing information and best practices on data systems and standards
        to evaluate outcomes, quality of care and system performance and
        support integration between health care providers to eliminate
        redundancy and enhance overall system efficiency

    The targets set out in the CHHS-AP are ambitious; however, much of the
work is already well underway. The CCN looks forward to working closely with
the CHHS-AP Steering Committee in the coming months to share knowledge and
resources so that we can improve the heart health of all Canadians and reduce
the growing burden and loss due to cardiovascular disease.
