the Exchange's adoption of certain new and revised forms, this is to advise
that effective June 1, 2007, the Exchange will only accept Personal
Information Forms ("PIFs") and Form 2C1 Declarations (the "Declaration") that
are prepared in accordance with the revised forms. Any PIF or Declaration
filed on or after June 1, 2007, that is prepared on the basis of the former
form will no longer be acceptable.
