Second Year Aging at Home Strategy Call for Proposals



    
    Toronto Central LHIN seeks innovative ways to help seniors live healthy,
    independent lives longer in their own homes
    

    TORONTO, Sept. 19 /CNW/ -

    NEWS

    The Toronto Central Local Health Integration Network today issued a Call
for Proposals in four priority areas to help seniors continue to live healthy,
independent lives in their own homes. Targeted for funding for the 2009/2010
fiscal year, submissions will address

    
    1.   Supportive housing services for seniors with dementia or
         mental illness
    2.   Increased access to specialized geriatric assessments in community
    3.   Crisis intervention for seniors with mental illness and/or
         addictions
    4.   Needs of seniors with dementia who are transitioning from acute to
         long-term care or home care.

    Deadline for submission is noon, October 17, 2008.

    Matthew Anderson, Chief Executive Officer, said:
    "The 2009/2010 Call for Proposals builds on the previous year's work and
     is focused on initiatives that
        -   will divert seniors from long-term care  homes and emergency
            departments of hospitals, or that
        -   will improve transitions for seniors from hospital care to care
            in the community.

    The goal is to develop a more integrated system of supports for seniors
    living in the community with chronic diseases and for those with
    dementia, mental illness and/or addictions. We want to improve
    satisfaction levels marginalized seniors with the care that they are
    receiving, while ensuring sustainability of our local health system."

    QUICK FACTS

       -   There are 130,697 seniors in Toronto Central LHIN
       -   $6.9 million was allocated to projects in 2008/2009
       -   $6.5 million is available to fund new projects in the
           2009/2010 year
