    VANCOUVER, Sept. 25 /CNW/ - Media representatives are invited to attend a
news conference this morning at St. Paul's Hospital.

    
    What:   The Canadian Patient Safety Institute will launch its
            fourth annual Canadian Patient Safety Week (CPSW) this morning at
            St. Paul's Hospital in Vancouver, B.C.

            Founded in 2005, the focus of this year's CPSW is medication
            reconciliation. The theme - Knowledge is the Best Medicine. Ask.
            Talk. Listen. - is meant to encourage patients, clients,
            consumers and all health care providers to become involved by
            asking more questions, talking with each other, and listening and
            understanding what is being said to help ensure medication
            safety.

            Representatives from Providence Health Care and the Canadian
            Patient Safety Institute will gather to raise awareness of
            patient safety issues and to highlight the important initiatives
            and innovations related to medication reconciliation happening
            across the country.

            Providence Health Care is recognized as a national leader in the
            area of medication reconciliation and was selected as the host
            site for the official launch of CPSW.
