$25 Million for seniors health initiatives in South West LHIN

    The South West Local Health Integration Network (LHIN) is receiving
$24,553,269 to ensure more seniors can continue to live independently and to
ensure patients receive care in the most appropriate setting.
    As part of the successful Aging at Home Strategy, the South West LHIN is
receiving $17,409,869 to help seniors receive needed health services in the
comfort and dignity of their own homes and communities. The Aging at Home
program combines traditional health services - such as home care and
supportive housing - with new, locally-driven, innovative approaches to caring
for seniors such as those listed in the accompanying backgrounder.

    
    The South West LHIN is also receiving:

    -   $4,896,500 for increased home care, personal support and homemaking
        services will be provided by the South West Community Care Access
        Centre (CCAC)
    -   $1,996,900 to invest in local solutions that will address alternate
        level of care (ALC) pressures
    -   $250,000 for nurse-led outreach teams to provide more care to
        patients in long-term care homes and help them avoid transfers to the
        hospital emergency department (ED)
    

    ALC patients are individuals in hospital beds who would be better cared
for in alternate setting, such as long-term care, rehab, or home. Having more
home care and community services enables ALC patients to leave hospital
sooner, making more beds available for patients that require hospital care.

    QUICK FACTS

    The South West Local Health Integration Network (LHIN) is a crown agency
responsible for planning, integrating and funding more than 150 health service
providers, including hospitals, long-term care homes, mental health and
addictions agencies, community support services, community health centres, and
the South West Community Care Access Centre. Established under the Local
Health System Integration Act, 2006, as one of 14 LHINs across Ontario, the
South West LHIN operates an annual health care budget of $1.9 billion. The
South West LHIN covers an area from Lake Erie to the Bruce Peninsula and is
home to almost one million people.

        $25 million for seniors health initiatives in South West LHIN
    

    The South West Local Health Integration Network (LHIN) is receiving
$24,553,269 in 2009/10 for a number of initiatives to help seniors live
healthy, independent lives in their own homes and to decrease the number of
alternate level of care (ALC) patients in hospitals.
    Having more home care and community services enable ALC patients to leave
hospital sooner, making more beds available to emergency department (ED)
patients and patients requiring hospital care. These services also provide
ongoing health supports to seniors which reduces their need to go the hospital
ED.
    ALC patients are individuals in hospital beds who would be better cared
for in alternate setting, such as long-term care, rehab, or home. Having more
home care and community services enables ALC patients to leave hospital
sooner, making more beds available to ED patients.

    
    Aging at Home Strategy
    ----------------------
    

    The South West LHIN is receiving $17,409,869 in 2009/10 to increase the
range and quantity of services available to seniors, and to help relieve
pressure in hospitals and long-term care homes.
    This year's investment is an increase of $10,404,263 from 2008/09. This
includes $7,612,164 in continued funding for initiatives approved in year one
of the strategy.
    The Aging at Home program expands traditional services that help seniors
stay healthy and live at home. These services include home care, assisted
living/supportive housing and end-of-life care.
    To date, the following initiatives have been approved for funding in
09/10. The remainder of the funding for this year will be allocated by the
South West LHIN Board of Directors at a public meeting later this year

    Increasing Home Care Services - CCAC Service Maximums
    -----------------------------------------------------

    The South West LHIN is receiving $4,896,500 in 2009/10 for changes made
last year to increase the availability and integration of home care services.
This included increasing the limits on hours of personal support/homemaking
services by 50 per cent, and removing limits entirely for hospital patients
waiting for a long-term care bed or receiving end-of-life services at home.

    Nurse-Led Outreach Team

    The South West LHIN is receiving $250,000 in 2009/10 for a nurse-led
outreach team that is being created to provide long-term care home residents
with timely and appropriate care, and stabilize residents who need more urgent
attention. This team of nurse practitioners and registered nurses will travel
to LTC homes to assess urgent problems, determine the need for hospitals care,
and provide interventions (such as intravenous therapy, antibiotic management
and administering oxygen) in cases where unnecessary visits to the hospital
and the ER can be avoided.
